Online Application Form for FCPS-II (Major Subjects)

  • Incomplete application form will not be considered.

FCPS-II (Major Subjects) application form has two steps.

Step-1 (Online Application Form)

  • A valid email address (preferably gmail) is required to complete the following procedure.
  • Select Registration type Fresh or Repeater.
  • Enter RTMC Number.
  • Confirm your email address and submit.
  • A verification code will be send to your confirmed email address.
  • Enter the verification code to proceeding step.
  • Complete the application form.
  • A pdf version of your form will be generated and sent to your email address.
  • After completing step-1, please process to step-2.



Step-2 (Submit form and required documents)

  • Before proceeding step-2 ensure that you have converted your all documents in PDF version and paid your fee.
  • Enter your Form Number and verification code.
  • Upload all required documents in PDF format and submit.
